Welcome to West Texas A&M University’s Athletic Training Online Participation Forms Portal.
We would like to thank you for taking part in our new process of accepting athletic participation paper work online. This new automated process will allow us to be more efficient in handling the forms as well as saving valuable resources.
• To access the online forms, you will need to click the “CLICK HERE” icon at the top, or “PROCEED TO ONLINE FORMS” at the bottom. It will take you to the Parent Portal.
• Once there you DO NOT NEED TO CREATE AN ACCOUNT. You can select Continue as guest.
• The forms that need to be completed will appear on the page. They will be listed under ELECTRONIC DOCUMENTS TO BE SUBMITTED BY THE PARENT/ATHLETE.
• There is five (5) forms. Insurance Card Upload, Student Athlete Medical Information, Concussion Form, Consent for Treatment, and Sickle Cell Test Release and Waiver.
• On the right hand side of the screen will be a tab “DOWNLOAD AND PRINT.” You will need to select that to get to the PHYSICAL and MEDICAL HISTORY.
• Click on the PHYSICAL FORM. This will contain the physical and medical history.
• All athletes new, returning, or transfer MUST FILL OUT ALL FORMS including getting a physical.
• Click on the form name and fill out the information requested. (You must have your Buffalo Gold Card number available, as it is required on each form)
o All athletes will complete:
• Student Athlete Medical Information
• Concussion Form
• Sickle Cell Test Release and Waiver
• Consent for Treatment Form
• Upload a copy of your Insurance card (BACK AND FRONT)
• Medical History
• Physical (Filled out by medical care provider, completed AFTER MAY 1st)
The General Medical portion of the physical examination
MUST be performed by a licensed physician of your choice PRIOR TO arriving at WTAMU.
• Take the Medical History Form and Pre-Participation Physical Evaluation form to the doctor.
• The doctor will fill out the Pre-Participation Evaluation
• The physical exam must be completed after
May 1st and before arriving at WTAMU.
• The completed physical must be turned into the athletic training staff prior to your participation in any athletic activities at WTAMU. This includes voluntary summer workouts.
• The
ATHLETE PHYSICAL (Medical History and Pre-participation exam) must be turned into the athletic training department before arriving at WTAMU. This can be done a few different ways:

• To sign the document click inside the signature box and hold your mouse down; this will allow you to create an "Electronic Signature". If you make a mistake and need to start over, click on the refresh icon next to the signature box.
• Make sure to put in a notification email and hit the submit button to save your form before advancing.
• Once you have filled out all of the information on each page you will have the opportunity to print the document, though it is not required.
• You will receive a confirmation email once the document has been reviewed by the WTAMU staff.
• Have a clear picture of your current insurance card ready to upload. Please label the file as shown:
